Anticipated Hiring Range:  $5,276 - $6,023 per month

Work Hours: Monday – Friday, 8am – 5pm, unless notified otherwise

 

 

THE DEPARTMENT:

The Care Center is a key component of the Campus Health and Wellbeing Cluster within the Division of Student Affairs at Cal Poly Pomona, embodying the University's commitment to holistic and preventative approaches to student wellbeing. In alignment with the University's mission and strategic goals, we are dedicated to creating a supportive space where students can flourish both academically and personally. The Care Center is the first point of contact for students in distress or of concern. We connect students to essential resources, provide advocacy, and support them through crises support. Our comprehensive Basic Needs services include Health and Wellness Products and resources, Mental Health Support Coordination, Housing and Financial Support, Food and Clothing Resources. By offering these diverse services, we aim to empower students, normalize the seeking of support, and build a community that prioritizes care, compassion, and student success.

DUTIES AND RESPONSIBILITIES:

CalFresh Program Implementation and Initiatives

  • Serves as the coordinator for all CalFresh programming and implementation – responsible for reviewing and processing incoming reports, using centralized database (Maxient), meeting with referred students via students’ preferred modality (i.e. in-person, zoom, phone), and creating action plan to address student needs
  • Provides personalized one-to-one care coordination/education to help students achieve their educational and career goals; which can include topics stress management, nutrition, substance use and health education and basic needs
  • Responds to student referrals from administrators, faculty, staff and other members of the campus community and provides feedback/follow up to referring parties where appropriate and on a need-to-know basis
  • Designs, implements, and evaluates programs/interventions for increasing awareness of CalFresh, health promotions to improve health knowledge attitudes and behaviors of students using Care/Wellness Center Services
  • Collaborate with key on and off campus stakeholders to develop a comprehensive, multidisciplinary, and holistic approach to student health and wellbeing
  • This position will serve as the primary point of communication for all CalFresh, nutrition and other food insecurity initiatives
  • Supports the implementation, reporting, and oversight of grants related to CalFresh and other food insecurity initiatives Implements selected recommendations for improvement and augmentation of the current CalFresh and basic needs services, particularly new initiatives

Outreach and Awareness

  • Utilizes Maxient (institutional care coordination information system) to schedule meetings and provide support and resources related to student's wellbeing
  • Maintains confidential student records and tracks student progress in electronic database
  • Promotes a community of care by assisting campus-wide trainings on CalFresh, health promotion basic needs, and other care related topics
  • Develops and delivers specialized training programs as requested by campus departments based on their needs, departmental trends, and experiences with students
  • Plans and implements publicity/marketing to promote the Care/Wellness Center and its services
  • Coordinates programs to support at risk students, including exploring new and innovative ways to engage students, faculty, and staff

Assessment & Evaluation

  • Conduct, analyze, and disseminate research data on the health and wellbeing of college students and prevention initiatives using existing data collection instruments and designing department specific instruments
  • Assists in the collection of assessment data; preparing reports and presentations of Care/Wellness programs and utilization
  • Serves on committees, task forces and working groups designated by the Director of Care Services to further the understanding of our student's needs

Other Duties as Assiqned

  • Performs other duties as assigned incidental to the nature of the position or appropriate to the missions of the Care Center, the Division of Student Affairs, and the university
  • Represent the department through committees, services on campus, community, and professional development identified by incumbent and agreed upon with supervisor
