Capital Health LIFE is a Program of All-Inclusive Care for the Elderly, one if the two original programs to have opened its doors over 15 years ago in New Jersey. It provides services to the residents in Mercer and Northern Burlington counties, who would otherwise be at risk of institutionalization. It offers a great opportunity to a physician to perform in a quality focused, value-based care model working alongside a team of clinicians while ensuring regulatory compliance.
Position Overview:
The incumbent will be highly productive physician passionate about providing high value care to the old and vulnerable. The scope of responsibilities includes but not limited to:
• Performs physical assessment of patients to identify priority care needs.
• Documents patient assessment and teaching and learning needs.
• Obtains detailed nursing history and identifies patients at physiologic and psychological risk.
• Consults with interdisciplinary team regarding patient assessment findings.
• Performs procedures necessary to implement plan of care based on care standards and protocols.
• Develops plan of care across the continuum and exercises discretion and judgement when revising based on physiologic and psychosocial findings.
• Orders tests and consults as required by plan of care and reviews test results.
Reviews patient plan of care with interdisciplinary team. Discusses plan with patient and family.
• Provides safe therapeutic care in holistic and systematic way to facilitate patients' achievement of desired outcomes.
• Demonstrates the ability to review and evaluate the plan of care for effectiveness of desired outcomes. Revises initial plans based on patient response to interventions.
• Documents all patient care activities as per documentation standards.
Evaluates outcomes of documentation and contributes to performance improvement initiatives through medical record review.
• Demonstrates effective communication skills and assists in the resolution of conflict among health care team members, patients, and families.
