We are offering a $5,000 sign-on bonus for new staff hired into this role full-time. This position is on site, and you have the option to elect into a 9/80 flex schedule. The pay range for this position is between $66,600 - $69,135 annually.

Summary

Responsible for:

  • Providing crisis intervention, intake, assessment, and individual, family, and group psychotherapy to transitional age youth (TAY) 15-25 years old (and their families, as appropriate) with a history of mental illness, substance use issues and/or social determinates of health needs.
  • Providing tailored clinical services to best meet the needs of the clients.
  • Offering Whole Person Care, including the integration of mental health, physical health and substance use treatment that is culturally congruent with our clients’ needs.
  • Engaging and outreaching to clients to promote full integration in the TAY drop in center, Our Third Place.

 

Primary Duties

  • Provides intake, assessment, crisis intervention, individual, group and family therapy; run therapeutic groups and facilitates conflict resolution in the center or field to clients ages 15-25.
  • Independently performs intake assessments and formulates diagnoses and case conceptualization using a culturally competent approach.
  • Demonstrates a collaborative approach within a multidisciplinary team and external systems.
  • Demonstrates ability to utilize a whole person/integrated care approach in assessment, treatment planning and intervention; assesses treatment progress and adapts interventions as needed.
  • Demonstrates competency in managing crises with minimal guidance.
  • Develops and implements clinical programming, such as therapy groups, for Our Third Place.
  • Uses collaborative documentation during therapy sessions in accordance with Agency standards.
  • Engages in measurement-based care through actively collecting and utilizing outcome measures to guide treatment; maintains outcome measures as required for agency/funding sources.
  • Assists with psychiatric holds for clients presenting a danger to themselves or others and/or are gravely disabled, and in need of hospitalization.
  • Provides some case management as needed, including: referral and linkages to internal and community support services, such as psychiatrists, primary medical care, housing, rehabilitation programs, drug/alcohol services and social activity groups.
  • Monitors and maintains timely documentation (including in the electronic health record, Power Apps, etc.) of client services; ensures compliance with Agency, county, state and federal policies and procedures, standards and guidelines.
  • Will carry a caseload of clients and meet Agency client service expectations as assigned.
  • Works primarily on-site, assists with client crises and with client walk-ins, building strong rapport and engagement.

 

Position Requirements

  • Current knowledge of all job specific skills including crisis intervention and individual/family therapy, multicultural and socio-economic issues, and documentation standards for the client population ages 15-25. .
  • The cultural humility and competence to work with a diverse population and help create a sense of belonging for clients engaging in the center.
  • Working knowledge of therapeutic intervention, assessment and intake for adults, children and families with a history of mental illness and/or substance use.
  • Depending on program/division, demonstrates working knowledge of Evidence Based Practices, Co-Occurring Disorders and/or the Recovery Model of treatment is required.
  • Knowledge of Agency policies and procedures, HIPAA, policies and documentation guidelines,
