About the role

The Home Infusion Pharmacist is responsible for facilitating full cycle pharmaceutical care specific to homecare patients, including but not limited to: medication review and management, pump and supply management, prescription fulfillment, coordination with intake and nursing agency teams, and delivery of the drug and supplies to the patient’s home.  

1.   Participates in patient evaluation and chart review for acceptance, product selection, device selection and supply management.
2.   Initiates infusion therapies by assessing the patient, developing a patient care plan and drug monitoring regimen and monitoring the patient according to the care plan throughout the course of therapy.
3.   Assists with the referral process including but not limited to: provision of medical information to patients and care givers relative to home infusion therapy; assistance in the teaching of patients prior to discharge to home; and coordination of deliveries with nursing and support services.
4.   Collects, interprets and evaluates all physician medication orders for therapeutic appropriateness, cost effectiveness, accuracy, safety and completeness.
5.   Evaluates and monitors the outcomes drug therapy relative to the total needs of the homecare patient by collecting, evaluating and interpreting the results of laboratory data and in collaboration with physicians, nurses and other healthcare professionals.
6.   Coordinates and supervises the drug preparation work of pharmacy technician and checks the accuracy of that work and all medications, supplies and equipment prior to dispensing from the pharmacy.
7.   When applicable, prepares specialty pharmacy home infusion products and aseptically compounds sterile parenteral/enteral pharmaceutical and chemotherapeutic agents for home infusion patients.
8.   Ensures compliance with all FDA, DEA, State Board of Pharmacy, OSHA and other regulatory standards.
9.   Ensures that all manufacturer requirements for REMS and/or other requirements are met when dispensing and/or shipping drugs.
10.   Completes accurate documentation of compounded and repackaged items. 
11.   Responsible for understanding of, compliance with and participation in Quality Assurance, Performance Improvement and accreditation guidelines ensuring achievement of excellence in quality patient care.
12.   Communicates with the patient, physician, nursing staff, pharmacists and other members of the health care team regarding patient status, response to therapy and clinical objectives to ensure clinical outcomes are met.
13.   Maintains working knowledge of all equipment and supplies necessary to administer therapies in the home or outpatient setting.
14.   Provides consultation and answers inquiries from patients, families, physicians and other healthcare professionals regarding drugs and their use.
15.   Completes and maintains thorough and appropriate documentation in the customer/patient record of all clinical activities and communication with patients, physicians and other healthcare professionals.
16.   Knows and adheres to all Mount Sinai home infusion and other health system policies and procedures and all applicable state and federal laws and regulations governing pharmacy practice.
17.   Participates and assists in inventory control, ordering of medications and supplies, warehouse organization, packing/shipping, equipment maintenance and hazardous waste control.
18.   Responsible for emergency on-call duties on weekends and weeknights as required.
19.   Learns and utilizes all computer programs necessary for the successful review, authorization, preparation, dispensing and management of home infusion patients.
20.   Remains familiar with literature, references and resources for therapies and care of patients serviced by the home infusion 
21.   Maintains the confidentiality of patients and their personal information at all times.
22.   Demonstrates effective communication with intake, patients, pharmacies, nursing agencies, warehouse / DME staff and other services in person and via telecommunications.
23.   Performs other tasks as assigned 
 

  • Bachelor of Science or Doctor of Pharmacy Degree from an accredited college of pharmacy.
  • Two years of experience in intravenous therapy in a hospital or in a home infusion setting that includes parenteral admixtures and patient clinical management
  • Teaching experience with nurses and home infusion patients 
  • Must be flexible with work schedule and emergency call duty
  • Minimum requirement of one year of previous experience in preparation of home infusion supplies and DME
  • Excellent aseptic technique must be demonstrated on an ongoing basis
  • Current and licensed pharmacist with the New York State Board of Pharmacy with ability to obtain additional licenses
  • Must be in good standing with all State Boards of Pharm
