MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

• Monday–Friday | Day Shift | No Nights, No Weekends, No On-Call
• Bilingual in Spanish Preferred
• $38/hr

• What You’ll Do:
• Provide direct nursing care and oversee participant treatment plans
• Review and update medical records; complete required documentation
• Support participants with medications and ongoing health needs
• Triage and manage care for ill or injured individuals
• Complete administrative tasks and ensure regulatory compliance

• What We’re Looking For:
• Graduate of an accredited RN program
• Active RN license in MA
• CPR/First Aid certification
