Job Summary:
Responsible for planning and organizing individual and group activities to meet the social, emotional, intellectual, and spiritual needs of the patients.
Qualifications:
Education/ Experience: High school graduate or equivalent. Proficient computer skills required. Ability to work independently carrying out diverse activities with minimal supervision.
Other Requirements:
- Experience in an inpatient setting or with Activity Therapy experience preferred. Able to lead and motivate people; excellent interpersonal skills.

Essential Functions Of The Position:
- Initiates referral process for patients once the provider places an order.
- Works with the Interdisciplinary Team on the creation and implementation of care plans and provides input as to each patient’s involvement in their areas or interest.
- Adheres to and conveys a philosophy that supports dignity, privacy, independence, choice, and individuality of patients.
- Conducts an assessment with each Swing Bed patient to determine those activities in which the patient would be most interested in pursuing and/or participating in. Provides one-to-one interaction with patients as needed.
- Plans and organizes individual or group programs, events, and activities around patient’s interests.
- Ensures that resident birthdays, and special accomplishments are acknowledged.
- Plans special “theme” days or months around holidays, seasons, or recognized days such as Father’s or Mother’s Day.
- Orders activity-related supplies as needed, and works in conjunction with the SS Director on the activity supply budget.
- Maintains the activity storage areas in a neat, clean manner. Picks up supplies after each activity and returns furniture to its original position.
- Coordinates uses of common areas with other disciplines, and checks resident appointment calendar to avoid conflicts.
- Demonstrates the ability to properly use the phone and computer systems, fax machines, photocopies, scanners, and printers.
