Location: Bridgewater, NJ (Somerset County)
Work Schedule: Full-time: Monday-Friday (3:00pm-11:00pm)

What You’ll Do:
• Support consumers in achieving personal care goals and addressing ongoing challenges.
• Transports consumers using the company van as needed.
• Ensure adherence to Adult Mental Health Rehabilitation Services (AMHRS) standards.
• Respond promptly to consumer concerns, ensuring high satisfaction.
• Educate consumers on medications, track logs, and conduct monthly audits.
• Coordinate services based on individual needs.
• Deliver high-quality care in line with Individual Rehabilitation Plans (IRP).
• Accurately document services within required timeframes using electronic health records.

Job Requirements:
• Bachelor’s Degree in mental health, psychology, sociology, or social work, or a related field from an accredited college or university OR
• A combination of 1+ years of college and related life or work experience totaling 4 years, OR
• High School/GED plus 4+ years of related work or life experience.
• Intermediate computer skills with the ability to complete documentation in an Electronic Health Record (EHR) system.
• Valid driver’s license and an acceptable driving record.
• Strong verbal and written communication skills.
• A compassionate, flexible, and client-focused approach to care.
