Job Description
We are seeking a Full-Time Nurse Practitioner or Physician Assistant to join our team and perform routine in-home medical assessments, follow-up visits, annual wellness visits, and acute primary care visits for complex, high-needs patients. This is a rewarding opportunity for a compassionate NP/PA who enjoys flexibility, autonomy, and building meaningful patient relationships outside of the traditional clinic setting.
Geographic Region:
Join VillageMD as a Nurse Practitioner or Physician Assistant in Houston, TX to cover a designated territory of Cypress, Tomball area.
Responsibilities:
- Conduct in-home patient visits for assessments, chronic disease management, medication review, and acute care needs
- Collaborate with an interdisciplinary team consisting of social workers, care coordinators, pharmacists, and supervising physicians
- Engage in goals of care conversations with patients and encourage completion of advance directives
- Ensure accurate clinical documentation of diagnoses to provide exceptional evidence-based care
- Provide chronic illness education to patients and families
- Participate in monthly team meetings to ensure adherence to quality standards
- Perform in-home wound care assessments as needed
Qualifications:
*Preferred clinical experience as a Nurse Practitioner/Physician Assistant in primary care, internal medicine, geriatrics, hospice and palliative care, or an in-home care setting.
Experience to drive change
- FNP/Family Nurse Practitioner, APRN, Nurse Practitioner or Physician Assistant/PA
- Current TX license
- Nurse Practitioner certification from either the ANCC / AANP
- Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS)
- Working knowledge of chronic care management and various medication options
- Current DEA licensure / Prescriptive Authority post-hire in TX
- Experience in Family Medicine, Internal Medicine, Geriatrics, Hospice and Palliative Care, or an in-home care setting
