Clinical Pharmacist — Essential Responsibilities

  • Provide clinical pharmaceutical care in alignment with the Patient Care Program.
  • Identify, prevent, and resolve medication-related issues through evidence-based, patient-centered therapies.
  • Verify prescriptions for accuracy and regulatory compliance.
  • Educate patients, caregivers, healthcare providers, and internal teams on disease management and safe medication use.
  • Identify high-risk patients and document clinical interventions.
  • Coordinate patient access to external healthcare services as needed.
  • Conduct drug utilization reviews and contribute to safety and performance improvement initiatives.
  • Report medication errors, adverse reactions, and recalls per protocols.
  • Support clinical program development and stay current on specialty medications.
  • Serve as Pharmacist-on-Duty when needed.
  • Perform additional duties as assigned.

Qualifications

  • Bachelor’s degree in Pharmacy or Pharm.D. with active pharmacist license in good standing.
  • Minimum 3 years of clinical patient care experience or relevant clinical rotations.
  • Strong knowledge of healthcare or specialty pharmacy operations.
  • Excellent communication, mentoring, and teamwork skills.
  • Analytical, organized, and able to manage multiple priorities.
  • Willingness to work extended hours, weekends, holidays, and participate in on-call rotations.
  • Proficiency with Microsoft Office and pharmacy software.

Preferred

  • Experience with compliance, regulatory, and accreditation standards.
  • Background in retail, mail-order, specialty pharmacy, PBM, pharmaceutical, or managed healthcare settings.

Work Environment

  • Ability to sit or stand for long periods and handle light lifting.
  • May involve exposure to warehouse settings, noise, and machinery.
  • Physical activities include bending, reaching, stooping, kneeling, and technical reading.
