Behavioral Health Clinician-HRSA Project

JOB SUMMARY: 

Collaborates and provides team-based biopsychosocial-spiritual healthcare and care coordination for patients who have coexisting mental health and psychosocial-spiritual challenges. This includes brief interventions and counseling for primary care patients including warm handoffs, behavioral health consults, individual/couple/family scheduled sessions, group visits/support groups, create concise, factual psychosocial-spiritual notes in the Electronic Health Record (EHR), and home visits. 

EDUCATION/EXPERIENCE: 

Bachelor's degree in a mental health related field, Master's degree preferred. One year integrated care experience in a primary or tertiary care setting highly preferred. Familiarity with Electronic Medical Records and integrated behavioral billing in primary care preferred. 

LICENSURE, CERTIFICATION, and/or REGISTRATION: 

Licensed Marriage and Family Therapist, Licensed Psychologist, Licensed Professional Counselor, or Licensed Clinical Social Worker. 

ESSENTIAL FUNCTIONS: 

1. Provides high-quality behavioral health services to patients served, in accordance with departmental and medical center policies and procedures, functioning within the professional code of ethics appropriate to licensure. Assists with Patient Health Questionnaire (PHQ-9) and Generalized Anxiety Disorder Questionnaire (GAD-7) screening of patients. 

2. Collaborates with Primary Care team members to provide holistic care for patients. 

3. Assists with mental health screenings of patients. 

4. Identifies and accesses local mental health resources for patients with needs. 

5. Identifies and links patients with community programs to meet housing/nutrition/transportation needs. 

6. Provides brief counseling for appropriate patients. 

7. Provides group visits/ support groups for clients. 

8. Acts as team resource to local agencies. 

9. Maintains mental health database of patient panel. 

10. Develops, manages, and maintains database for population health tracking. 

11. Maintains database of identified patient needs and resources. 

SKILLS/QUALIFICATIONS: 

Knowledge of the principles and techniques of administrative management skills including organization, planning, staffing, training, budgeting, and reporting Moderate proficiency in Microsoft Excel and Word Familiarity with Electronic Medical Records and integrated behavioral billing in primary care WORK ENVIRONMENT: Subject to interruptions
