Clinical Care Coordinator - Primary Care Shared Services

The Clinical Care Coordinator coordinates services for patients, focusing on those who face particular difficulty with navigating the healthcare system, management of specialty and home health referrals, and supports patients in self-care and identifying relevant community resources.  This position collaborates with internal and external healthcare fields to provide seamless transitions of care, while providing patient education and assisting with access to applicable resources.

Required Qualifications: 

• National Provider BLS - American Heart Association upon hire.

Preferred Qualifications:

• Associate's Degree
• 2 years of Care coordination experience.
• 1 year of experience in an outpatient setting.

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
