Licensed Practical Nurse - Primary Care - Per Diem

This position requires coverage (float) at our 8 primary care and family medicine clinics in the area. Transportation and flexibility are a must. Shifts will be 8 am - 5 pm, Mon-Fri, with potential weekend opportunities. All locations include onsite parking.
Per diem LPN's will be paid the following per diem shift differentials on top of their base pay for every hour they work:
$2.00
Per Diem - Day
$4.50
Per Diem - Evening
$6.00
Per Diem - Night
$8.00
Per Diem - Weekend - Day
$10.50
Per Diem - Weekend - Evening
$12.00
Per Diem - Weekend - Night
JOB DESCRIPTION:
The Ambulatory LPN – Primary Care is an integral part of the care team and a key contributor to delivering high quality care that is compassionate, consistent and reliable for our patients, families and the practices within a medical home. The incumbent is involved in direct patient care under the direction of a provider, provides technical nursing care to patients in varying states of health and illness and performs clinical and administrative health care support services to patients and families according to knowledge, ability, and duties assigned.
EDUCATION:
Current LPN licensure or compact licensure recognized by the Sate of Vermont is required. CPR certification required within the first six months from hire.
EXPERIENCE:
One to two years' experience in relevant health care setting or training program desired. Primary Care experience preferred. Proven customer service experience required.
