Job Summary:

Allina Health is seeking a Licensed Marriage and Family Therapist (LMFT), Licensed Independent Clinical Social Worker (LICSW), or Licensed Professional Clinical Counselor (LPCC) to join our Adult Partial Hospitalization Program (PHP) team in Faribault, MN. This role focuses on providing evidence-based therapy, diagnostic assessments, and individualized treatment planning for adults in a structured outpatient treatment setting.

Key Position Details:

As a Mental Health Licensed Therapist (LMFT, LICSW or LPCC), you’ll perform mental health diagnostic assessments and provide individual, family, and group therapy within established programs. Responsibilities include developing treatment plans, coordinating care with community providers, and supporting patients through their course of care. For Intermediate Care Programs, practice aligns with CMS, MN DHS Manuals, and MN §245I standards.
