A Community Health Worker contributes to AHF by working to create connections between our vulnerable population and their AHF healthcare center providers. The community health worker also conducts extensive outreach to re- engage at risk patients back into care and works to retain them in care in order to achieve desirable health outcomes, regardless of payor source.
A Community health worker enhances the reputation of AHF by providing high quality, patient facing service. You will be a passionate advocate for our top initiatives. To be successful as a Community health worker it is crucial to ensure that the AHF Core Values and Mission stay top of mind with all that you do. AHF has a collaborative organizational structure where staff are accountable to multiple leaders.
The Community health worker will work as a part of a close-knit team to keep patients engaged in their healthcare and assist in navigating and eliminating barriers that prevent them from successfully engaging in care.
Licenses and Certifications:
High school/equivalency diploma required. Associate/Bachelor’s degree Public Health specializing in community health, social services, or related field preferred. Three years related experience and/or training; or equivalent combination of education and experience.
