Care Coordinator - Non Clinical (Specialty)

About the role

We are seeking a Care Coordinator, non-licensed, to join our Delaware Valley team in a hybrid position! The Non-Licensed Care Coordinator will coordinate ambulatory team-based care to provide comprehensive support and guidance to patients with chronic diseases that require specialty care through effective partnerships with the PCP, community resources, medical professionals, the patient’s medical home team, and the Nemours organization to enhance patients’ access to education, counseling, and ongoing support beyond the clinical setting. 

They assist patients and families in meeting medical and psychosocial needs and promoting health and well-being. The Specialty Care Coordinator will collaborate with specialty staff to identify patient and family needs and address potential barriers such as no-shows, social drivers of health (SDOH), and pre-visit planning. This role is crucial in ensuring that patients receive timely and appropriate care, improving health outcomes, and enhancing the overall patient and family experience. They address gaps in care and promote timely access to appropriate care, increasing the utilization of preventative care and healthy behaviors to support the Nemours priority of whole child health.

Responsibilities:

  1. Collaborate with specialty staff to identify patient and family needs related to access to care.
  2. Address barriers to care, including no-shows, social drivers of health SDOH, and pre-visit planning.
  3. Ensure patients complete all required labs and screenings before their visit.
  4. Act as a liaison for agencies and families with identified healthcare needs.
  5. Assist families and patients through the healthcare system by acting as a patient advocate and coordinator of care.
  6. Connect patients to relevant community resources to enhance patient health and well-being.
  7. Serve as a point of contact, advocate, and informational resource for family, patient, care team, school systems, community resources, and state agencies.
  8. Facilitate meetings/calls between patients/families, care teams, payors, and outside agencies as needed.
  9. Reinforce education to patients and families about conditions (existing or newly diagnosed) to assist them in appropriate self-management.
  10. Participate in data collection, health outcomes reporting, audits, and program evaluation related to whole child health.
  11. Serve as contact point, advocate and informational resource for family and community partners/payors.  Referrals to child protective services and appropriate agencies for domestic violence.  Completes forms such as DFS, FMLA, SSI, etc. and writes letters for housing, nursing care, medical necessity, etc.; Research, find and link resources, services, and supports with/for the patient/family.  Arranges for supplies and equipment. Make referrals to Nemours Financial Assistance Department and support procurement of patient health insurance, as needed
  12. Additional miscellaneous duties and responsibilities, as may be assigned from time to time by employee’s supervisor.

Qualifications:

  • High School Diploma required 
  • Some College level course completion preferred and or prior experience 
  • Scheduling, Insurance, SDOH resources, pediatric experience preferred
