Responsible for assessing, planning, coordinating and evaluating the care of patients.Job Description
Primary Duties & Responsibilities:
- Communicates with patients daily about service needs, service guidelines and eligibility.
- Communicates with medical and supportive service providers/staff daily regarding scheduling, client/patient barriers, best practices, etc.
- Monitors inventory, determines needs, and places orders for client service supplies (i.e., food pantry, toiletries, bus tickets, etc.).
- Tracks timeliness of services provided; reports and addresses issues with services, troubleshooting and resolving issues.
- Completes service documentation in EPIC and statewide case management database.
- Monitors and requests additional supplies from vendors and ensures timely receipt and distribution.
- Conducts a comprehensive sensitive assessment with the client and family/support network as appropriate, in order to develop a plan of care to meet identified needs.
- Arranges and coordinates service referrals for the client/family as needed; provides ongoing monitoring of the activities implemented to carry out the plan of care. Refers clients/families to public benefits programs as eligible.
- Provides regular contact with enrolled clients and documents all contacts and service coordination activities in client records as required by program guidelines.
- Educates clients/families about the basic medical and psychosocial aspects of disease in order to optimize adherence to care.
- Provides proactive follow up, tracking symptoms and treatment responses.
- Adheres to all applicable state laws and the department’s rules and regulations regarding programs.
- Stay abreast of new resources, programs and developments. Participates in continuous training and education, including attendance at staff meetings and community meetings as assigned.
- Other duties as assigned.

Required Qualifications:
- Basic Life Support certification must be obtained within one month of hire date.
- Basic Life Support certification (Online BLS certifications, those without a skills assessment component, are not sufficient to meet the BLS requirements).
