Adventist HealthCare seeks to hire an experienced Outpatient Care Navigator for our Care Navigation Department who will embrace our mission to extend God’s care through the ministry of physical, mental, and spiritual healing.
As a Care Navigator you will:
• Identify, assess, plan, implement, coordinate, monitor, and evaluate healthcare services to meet an individual patient's specific health care needs in a cost-effective manner. (may include but not limited to home health assessment, telehealth initiation, disease management education, coordinate of post-acute services)
• In collaboration with the Community Health Worker, coordinate patient care with patient, family, staff, physicians, and ancillary/community services to promote quality and cost-efficient care for assigned caseload.
• Ensure weekly contact with patient (in person or via telephone) to progress towards care plan goals
• In collaboration with the Community Health Worker, act as an advocate for the connection with community resources.
• Conduct patient home assessment visit when indicated
Qualifications include:
• Graduate of an accredited school of nursing with a minimum of an associate of science degree required, bachelor’s degree preferred
• 1 - 3 years’ experience in acute care setting at least 1 year of prior experience as case manager, discharge planner or care coordinator required
• Maryland RN licensure required
• Licensed driver and able to operate a motor vehicle as well as maintain automobile insurance required
• Active American Heart Association Basic Life Support (BLS) certification required
Work Schedule:
Monday-Friday. Occasional Holidays as needed
Pay Range:
$77,313.60 - $115,980.80If the salary range is listed as $0 or if the position is Per Diem (with a fixed rate), salary discussions will take place during the screening process.
