POSITION SUMMARY

The Registered Nurse-Hospital at Home Float Nurse is a strong individual on the health care team who will render quality skilled nursing care to a range of high-acuity to low-acuity clients in transition from hospital to home on a short-term and as needed basis. The Registered Nurse-Hospital at Home Float Nurse will serve as a flexible and critical team player along with preceptor to BAYADA Home Health Care field nurses, providing training to BAYADA home team in preparation for ongoing, permanent home-care team.

MINIMUM QUALIFICATIONS:

Each Registered Nurse must:

  • Have a current license as a Registered Nurse in the state in which he or she
  • Have at least one year of verifiable work experience as a professional
  • Demonstrate competence in performance of following skills, as applicable:
    • Waived testing, g. glucometer and PT/INR (demonstrated in home or lab)
    • Tracheostomy/ventilator care (demonstrated in home or lab)
    • Infusion experience
  • Demonstrate an ability to read, write, and effectively communicate in
  • Achieve a passing score on high tech Adult and Pediatric
  • Hold current CPR certification if providing adult and pediatric nursing services in the Skilled Nursing Unit. NOTE: State regulation and/or office specific policy may exceed this requirement.
  • Pediatric Advanced Life Support (PALS) and Neonatal Advanced Life Support (NALS) preferred but not
  • Ability to work in a teaching setting to prepare and educate families and BAYADA Care team for the transition from hospital to home.
  • Have a criminal background check completed in accordance with BAYADA policy & for each state licensed.
  • Possess flexible qualities and the ability to maintain cohesiveness among all team
  • Possess strong critical thinking skills to work effectively and independently in the
  • Demonstrate strong inter-personal communication skills to maintain relationships with the health care team, family, payor sources, and HICU® /service office team members.
  • Interact as an extension of the HICU® care team and BAYADA care teams to facilitate positive long-term relationships to represent BAYADA as the preferred home care provider for complex, chronically ill patients.
  • Act as mentor to BAYADA field nurses by providing guidance and support in the development of their nursing skills to ensure continuity of care for clients and their families
  • Maintain all equipment and ensure supplies are available and working
  • Maintain a flexible schedule of availability and exceptional
  • Establish and maintain relationships with multiple local BAYADA field
  • Flexibility to accept last-minute shifts to achieve goal of 100% reliability in client
  • Promote effective care coordination with the facility and communicate clinical status and staffing needs to the service office and HMC/VMC to ensure high quality and safe transition to the home care clinical while representing BAYADA with a professional demeanor.
  • Travel expected within a set timeframe distance to serve multiple service

PRIMARY RESPONSIBILITES:

  • Adhere to the established client care plan and the Physician’s Plan of Treatment.
  • Performs assigned duties, including administration of medications, treatments, procedures as indicated, monitoring the client’s condition, etc.
  • Understands and follows safety and infection control policies and practices.
  • Reports any changes in the client’s condition to the Clinical Manager, Physician and Senior Associate, SNS; follows up with executing and properly documenting any new doctor’s orders.
  • Makes ongoing additions to the client’s care plan, as needed.

  • Provides detailed and appropriate teaching and education to families and BAYADA Care Team to effectively guide them through the care plan as well as transition to the home.
  • Serve as nurse preceptor and clinical lead in the home to the BAYADA Care Team by educating on new skills quickly and effectively to ensure the provisioning of ongoing quality care.
  • Available to participate in discharge planning and care plan meetings in the facility setting and home setting.
