About the role

Job Summary: The Community Health Worker II (CHW II) provides culturally responsive, trauma-informed support to birthing individuals and families across the perinatal and postpartum continuum. This role functions both within the postpartum inpatient unit and aboard the Community Mobile Health Clinic, serving as a vital link between clinical care and community-based services to promote maternal and infant wellness.

The CHW II collaborates closely with clinical and non-clinical staff, multidisciplinary teams, and community partners to ensure high-quality, patient-centered care. This role brings a strong foundation in community engagement, safety awareness, and perinatal / postpartum health equity, prioritizing the voices and lived experiences of the families served

Specifications: High School or GED Required. ??????Driver’s license, reliable transportation, and current auto insurance. 1–3 years of relevant experience in healthcare, maternal-child health, or public health Experience in hospital, security, or crisis settings preferred. Bilingual in English and Spanish or another language relevant to the community served preferred.

Required certifications: Community Health Worker Certification (or willingness to obtain within 6 months of hire)

Preferred certifications: Lactation support certification (e.g., CLC, IBCLC, or equivalent), Mental Health First Aid or peer support certification, Commercial Driver’s License (CDL) or equivalent.
