Home Health PT

Minimum Qualifications:

• Bachelor’s degree with progressive experience or a Master’s degree in Physical Therapy
• Current license to practice as a Physical Therapist in Ohio
• Working knowledge of Conditions of Participation and home healthcare experience preferred
• Valid driver’s license and reliable transportation
