Overview
Provides direct bereavement care to the surviving family members and significant others of Hospice clients according to the bereavement plan of care developed by the Bereavement Counselor and Interdisciplinary Team. Ensures that services provided are in compliance with Hospice and Hospice Care philosophy and procedures. Works under general supervision.Compensation Range:$62,400.00 - $72,000.00 Annual• Implements the formal program of bereavement in accordance with the CMS Conditions of Participation. • Attends weekly team meetings as a member of the interdisciplinary team. • Monitors assessment for bereavement risk of the primary bereaved family member(s) from time of admission. • Provides appropriate support prior to the patient’s death as per Interdisciplinary team (IDT)request • Educates IDT regarding anticipatory grief and mourning. • Completes assessment of primary bereaved family member(s) at time of death of the patient. • Updates bereavement assessment and bereavement plan of care throughout time of service. • Facilitates bereavement counseling group sessions and workshops to achieve effective grieving. • Conducts workshops and other education programs related to dying, death and bereavement. • Documents counseling provided in the client's clinical record in accordance with State and Federal regulations and Hospice policy. • Evaluates the progress of family members and significant others one year after the death of the Hospice client and determines the need for continued bereavement counseling and community clients. • Provides bereavement support (telephone, face to face, support groups, workshops) to community members as needed and as program needs allow. • Collaborates with Manager in the development of the Bereavement Support Program. • Acts as liaison with the Hospice Team when the Manager is not available. • Offers individuals of different philosophies and religious perspectives opportunities to discuss and share their thoughts, feelings, beliefs and values. • Participates in special projects and performs other duties as assigned.Qualifications
Licenses and Certifications:License to practice social work, psychology, or related field in New York State Preferred Valid driver's license may be required, as determined by operational/regional needs.
Education:
Master's Degree in Social Work, Mental Health Counseling, Thanatology, Psychology, or related field Required
Work Experience:
Minimum one year of experience working with terminally ill patients and bereaved families in a hospice or palliative care setting Required Competency in bereavement counseling required including both individual and group counseling. Required Proficiency in personal computer operations including MS Office applications Required Bilingual skills may be required, as determined by operational needs. Preferred Prior experience leading both individual and group counseling Preferred
