Summary
The Housing Stability area of Human Services Public Health is seeking two Case Management Assistants to join the Homelessness Prevention team to support residents remaining in or newly accessing stable housing and avoid homelessness. These positions will support the Homelessness Prevention team’s eviction prevention strategy, and will provide triage services at housing court, respond to calls from residents needing assistance to maintain their housing, provide navigation and support to residents with housing instability, and support continuous improvement projects with the team and Hennepin County.

About these position types:
These are limited duration (temporary) full-time, benefit earning position expected to last up to two years.
These positions are internally classified as Case Management Assistants.

- Provide triage and navigation for residents at risk for eviction, including resource referrals for legal representation, benefits, or supportive services.
- Assist residents with initiating or completing applications for emergency rent assistance.
- Maintain respectful and trauma-informed customer service practices.
- Follow Rent Help Hennepin eligibility policies, keeping up to date on policy and procedure changes.
- Complete triage forms and refer residents to appropriate resources.
- Communicate in a professional and timely manner with customers and housing partners.
- Respond to application inquiries from resident and property managers.
- Follow team communication guidelines when working with community partner agencies.?
- Attend all required one on one, unit, and program meetings.
- Contribute to a positive, inclusive work environment supporting team members, colleagues, and customers.?
- Complete other special projects, as assigned.
- One of the following:
- Two years or more of responsible, paid social service experience providing direct client service.
- A two-year human services certificate.
- Two years or more of college coursework in the behavioral sciences.
