🧠 What You'll Do:

• Assist with resident admissions, discharges, and care planning

• Support residents and families through transitions and emotional needs

• Coordinate with nursing, therapy, and administrative teams to ensure holistic care

• Maintain accurate documentation in accordance with state and federal guidelines

• Help plan and implement programs that enhance quality of life

• Advocate for residents’ rights and help resolve concerns compassionately

What We're Looking For:

• A heart for service and empathy for residents

• Experience in social services or a related field (preferred)

• Strong organizational and communication skills

• Familiarity with long-term care or skilled nursing environments is a plus
