General Position Description: Supports Nurse Home Visitors serving pregnant women, families, and young children, to successfully implement the respective evidence-based service model (Maternal Early Childhood Sustained Home-visiting Program (MECSH), Nurse Family Partnership (NFP)) in accordance with model standards and funder requirements.
Program Summary: Nurse home visiting includes in-home comprehensive care for families starting during pregnancy and continuing until the child is a toddler. Nurses provide proactive counseling and ongoing support for families centered on the following topics: child health and development, maternal well-being and mental health, personal and community relationships, family aspirations, and goal setting. Nurses also provide developmental screening, and basic health monitoring of the mother and child.
Essential Duties and Responsibilities:
- Maintains good standing and current affiliation with the Nurse Family Partnership and/or Maternal Early Childhood Sustained Home-visiting (MECSH) model(s).
- Ensures program implementation meets fidelity to the respective model’s standards in regards to enrollment, documentation, frequency of visits, screening and assessments, transition planning for families, staff training and certification, and staff education requirements.
- Monitors caseloads to ensures contract service units are met and that the Nurse Home Visitors do not exceed model caseload requirements.
- Evaluates program areas and service needs on a regular basis according to agency, funding, and quality assurance standards.
- Provides support to staff and participates in daily activities essential to the completion of program objectives, including but not limited to: family assessment, home-based family support, referral and coordination, case plan development, and documentation of services.
- Monitors that required data is collected, reported and synthesized in order to reflect on program implementation and report data accurately and timely. Completes funder and model reporting requirements, including narrative and data reports. Reviews monthly invoices to funder.
- Facilitates training and in-service related to best practices in nursing and early childhood programs. Participates in community opportunities to promote and explain the respective nurse home visitation program goals and services to facilitate referrals and the importance of early intervention.
- Provides strength-based, reflective supervision with each nurse home visitor.
- Provides a broad range of administrative and supervisory functions. Supervises assigned staff in accordance with EBF personnel practices including recruitment, training, performance evaluations, and day to day scheduling. Participates in the process of ongoing professional development. Maintains comprehensive communication among all contacts within the agency, members, and external agencies. Models professional standards and ethics in accordance with EBF Code of Conduct, Strategic Plan, Mission, and Vision.

Requirements
Minimum Requirements:
- Bachelor’s or Master’s degree in Nursing with 5 years of experience working with at-risk women, their children and extended families and 1 year of supervisory experience required.
- Bilingual (Spanish) preferred.
Regulatory:
- Must be at least 21 years of age.
- Current, unrestricted AZ driver’s license with no more than two (2) minor moving violations or one (1) accident within the past three (3 years). Three (3) years of driving experience required.
