About the role
We are searching for a Pharmacist to join the team! The Pharmacist for Pharmacy Outreach will be responsible for enhancing patient care through telephonic outreach assessments and interventions. This role will focus on activities such as polypharmacy, medication adherence, drug-drug interactions, high-risk medication prescribing, pharmacotherapy optimization, and disease state management.

Essential Duties & Responsibilities:
- Identify and address patient-specific barriers to medication adherence and disease state management.
- Prevent, detect, and resolve medication-related problems and make appropriate evidence-based, patient-centered medication and therapy recommendations
- Collaborate with providers, clinical and administrative staff to optimize the Medication Management workflow and patient experience
- Adjust and recommend alternative pharmacotherapy based on formulary changes.
- Assist providers, care managers, or other members of the practice team with management of high-risk, high-cost patients to ensure optimal patient care.
- Other duties as assigned
Required Skills and Abilities:
- Strong communication skills with patients, physicians, and other healthcare professionals
- In depth knowledge of pharmacotherapy and pharmacy practice
- PGY1 pharmacy residency
Required Education & Experience:
- Graduate of an accredited School of Pharmacy
- Doctor of Pharmacy (PharmD)
- 1+ year of experience in a clinical pharmacy setting
- Current and valid multi-state licensure including at least 2 of the following states: AR, DE, OH, KY, MS, PA, WA, WV
- Willingness to gain up to 3 additional licenses within 90 days of hire
Preferred:
- Licensure in 4+ of the above states; BCACP
