BAYADA Home Health Care is offering a unique opportunity for a Registered Nurse, RN with an entrepreneurial spirit who is interested in helping to grow a new territory for our existing Home Health Division in Northeast/Central PA and Ohio, as we enter the Western PA market in Erie.
This is a great opportunity to participate in a structured career path to advance in the Home Health Field!
In this role you would initially learn all aspects of the expectations of a BAYADA field case manager and support a small case load in Erie County, prior to transitioning to an education specialist/preceptor or office based clinical manager role.
You will be a key presence in a new territory, giving you an opportunity to build relationships with our top accounts and facilitate growth strategies that will result in expanding our service area and providing top quality care to more clients!
This position ultimately presents the opportunity for a motivated individual to initially grow client census and, if interested, could eventually support and manage your own office as an operational leader, joining a team of like-minded directors who currently support our Home Health Practice in over 100 other Bayada Offices from Florida to Hawaii.

Responsibilities:
- Directing and coordinating the care provided by a multi-disciplinary team of home health care professionals, including RNS, PTs, OTs, SLPs, MSWs and HHAs
- Management and mentoring of Clinicians
- Orientation and ongoing training and education of clinical professionals
- Documentation review, including OASIS & ICD-9 (10) coding
- Monitoring quality assurance
- Managing staff productivity, and overall case management.
- Visiting clients as necessary
- Lead, inspire and motivate others to provide exceptional care
- Demonstrate the ability to build relationships with employees, referral sources, and community resources while growing the office
Qualifications:
- Proven experience in home health care is strongly preferred
- Medicare Certified Home Care Agency experience is strongly preferred
- In-depth knowledge of Home Health regulations, OASIS and Medicare Conditions of Participation for home care is preferred
- Understands and connects with The BAYADA Way
- Strong interpersonal, communication, and educational skills

Registered Nurse (RN) Responsibilities:
- Follow a designated care plan in accordance with patient's needs
- Make home visits to clients in designated geographic territories
- Perform assigned duties, including administration of medication, wound care, treatments, and procedures
- Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
- Follow up with, execute, and properly document doctors' orders
- Perform client assessments as necessary
- Case management and coordination
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet
