As a Family Medicine Physician (MD/DO), you will help individuals and families access high-quality, comprehensive healthcare by providing preventive, acute, and chronic disease management services that improve health outcomes and strengthen the communities we serve.

Your work will directly support improved patient outcomes, better access to care, and the overall health and stability of the communities we serve.

Why You'll Love This Role

This role offers the opportunity to practice meaningful community medicine in a mission-driven healthcare environment where your clinical expertise directly impacts patients who may otherwise face barriers to care.

You'll be part of an integrated care team that values collaboration, patient relationships, and whole-person health while providing comprehensive care to patients across the lifespan.

Highlights of this opportunity include:

  • Collaborative practice environment with physicians, advanced practice providers, nurses, behavioral health providers, care coordinators, and community health workers.
  • Ability to build long-term relationships with patients and make a meaningful impact in the communities we serve.
  • Supportive team-based model focused on quality care, access, and improving health outcomes.
  • Opportunity to participate in clinical leadership, mentorship, and quality improvement initiatives.

What You'll Do

In this role, you will:

  • Provide comprehensive primary care services for patients across the lifespan, including preventive care, acute illness management, and chronic disease management.
  • Conduct patient assessments, diagnose conditions, develop treatment plans, prescribe medications, and coordinate referrals as needed.
  • Build strong relationships with patients through education, counseling, and shared decision-making.
  • Collaborate with an interdisciplinary healthcare team to support integrated, whole-person care.
  • Maintain accurate and timely documentation while supporting quality measures, patient safety, and regulatory requirements.
  • Participate in quality improvement initiatives, peer review, and clinical activities that enhance patient care.

What We're Looking For

We are looking for someone who is compassionate, community-minded, and committed to improving access to care. The ideal candidate is collaborative, reliable, and motivated by meaningful work that supports patient and community health.

The ideal candidate:

  • Provides evidence-based, patient-centered care while building trusting relationships with individuals and families.
  • Enjoys working in a collaborative, team-based environment with healthcare professionals across disciplines.
  • Demonstrates strong clinical judgment, adaptability, and commitment to serving diverse patient populations.
  • Values preventative care, health equity, and improving access to healthcare services.
