Position Description:

The Substance Use Care Coordinator enhances the quality of patient management, maximizes satisfaction and promotes cost effectiveness.  The Substance Use Care Coordinator will be accountable for the patient care cycle and is expected to assess, plan, implement, monitor and evaluate actions required to meet the client’s health and human services needs.   


Duties and Responsibilities: 


Coordinate and provide care that is safe, timely, effective, efficient, equitable, and patient-centered.
Helps patients achieve wellness and autonomy.
Facilitate multiple care aspects (case coordination, information sharing, etc.).
Helps patients make informed decisions by acting as their advocate regarding clinical status and treatment options.
Develop effective working relations and cooperate with medical team, clinical team throughout the entire case management process.
Arrange abeyances with social services, health and governmental agencies.
Be proficient with State Licensure Standards, CARF Standards, 42 CFR and HIPAA standards and Company policies
Adhere to professional standards as outlined by protocols, rules, and regulations.
Performs other duties as requested by the Clinical Director.

Qualifications
Bachelor's Degree in related healthcare field or social services
Experience in working in case management.
Proficient in the use of personal computers
Excellent oral and written communication skills
