40Summary:
This position coordinates collections and analysis of data to support monitoring and improvement activities within CCHH. This person will support functions to provide statistical reports related to clinical service metrics. In addition, this position will be called upon to facilitate improvement teams, chair groups, and will be expected to work closely with the department leaders of CCHH on clinical and operational quality and utilization management issues within CCHH. This position is also integral in the identification and correction of CCHH regulatory and contractual compliance issues.
Education and Experience:
Minimum Licensed Vocational or Registered Nursing degree with minimum of two years of experience in quality management application and support, reporting, and CQI methodology
Home Health experience with sound understanding of CMS regulatory and Joint Commission standards regulatory compliance highly desirable
Certification/Licensure:
Licensed Vocational Nurse or Registered Nurse Required
Quality Management Certification desired
