Clinical Liaison *ESIM sent 10/21

The Clinical Liaison position is responsible for creating and maintaining positive customer relationships in order to increase referrals for Home Health, Hospice, Home Infusion, PACE, Transitional Care Unit and other services within assigned accounts.  This role includes working with discharge planners, facility staff, physicians, and individuals to assess appropriateness for hospice and home health services, and facilitate the coordination of services and to ensure there is appropriate documentation for compliance with agency and statutory guidelines to manage the transition of the patient from an acute care facility to the patient's home or next location of residence.

Required Qualifications:

• Education sufficient for clinical licensure for discipline (e.g., LPN LVN, Registered Dietitian) Or
• Bachelor’s Degree in Social Work.
• Applicable clinical discipline (e.g., Respiratory Therapy) upon hire.
• 3 years experience in clinical discipline.

Preferred Qualifications:

• Social Work in Social Work
• Bachelor’s Degree in Applicable clinical discipline (e.g., Respiratory Therapy).
• Liaison, marketing or sales experience.
• Health care industry experience.
• Documented prior successful sales account medical management experience.
• Epic EMR experience.

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
