- Conduct comprehensive patient assessments to evaluate nutritional needs based on medical history and current health status.
- Develop, implement, and monitor individualized care plans that include dietary recommendations and interventions.
- Monitor patients' progress and adjust meal plans as needed
- Educate patients on proper nutrition, including food choices, portion sizes, and meal timing
- Collaborate with healthcare professionals to ensure comprehensive patient care
- Utilize physiology knowledge for effective dietary recommendations
- Perform diagnostic evaluations to assess patients' nutritional status
- Offer health coaching to support patients in achieving their dietary goals - Develop preliminary and comprehensive assessments of the dietary needs of each resident.
- Provide nutrition expertise and serve as a resource to other members of the health care team.
- Participate in the planning, conducting, and scheduling of timely in-service training classes that ensure a well-educated dietary service department.
- Maintain current knowledge in clinical practices of nutritional therapies and treatment protocols.
- Ensure that all dietary personnel are aware of the Care Plan and that Care Plans are used in planning daily dietary services for the residents.
- Review nurses’ notes and meets with Direct Care Staff to determine if the Care Plan is being followed.
- Discuss problem areas with the Director of Nursing.
- Review and revise Care Plans and assessments as necessary, at least quarterly and annually.
- A degree in Dietetics or a related field is required; completion of an accredited internship is preferred.
- Previous experience working in a hospital/long term care setting is highly desirable.
- Strong knowledge of physiology as it relates to nutrition and health management.
- Familiarity with dietary systems used for patient management within healthcare facilities.
- Excellent communication skills for effective interaction with patients, families, and healthcare team members.
- Ability to work collaboratively in a fast-paced environment while maintaining attention to detail.
- Other duties may be assigned.
- 8 hour shift - 9am-5pm
- Day shift
- Monday to Friday
- Expected hours: 40 per week
- Pay Range: $50-55/hr
