OVERVIEW We are seeking a dedicated and knowledgeable Dietitian to join our healthcare team. The ideal candidate will possess a strong background in dietetics and physiology, with the ability to assess patient needs and develop tailored nutrition care plans. This role is essential in promoting health and wellness through effective dietary management within a long-term care setting.FULL JOB DESCRIPTION

• Conduct comprehensive patient assessments to evaluate nutritional needs based on medical history and current health status.
• Develop, implement, and monitor individualized care plans that include dietary recommendations and interventions.
• Monitor patients' progress and adjust meal plans as needed
• Educate patients on proper nutrition, including food choices, portion sizes, and meal timing
• Collaborate with healthcare professionals to ensure comprehensive patient care
• Utilize physiology knowledge for effective dietary recommendations
• Perform diagnostic evaluations to assess patients' nutritional status
• Offer health coaching to support patients in achieving their dietary goals - Develop preliminary and comprehensive assessments of the dietary needs of each resident.
• Provide nutrition expertise and serve as a resource to other members of the health care team.
• Participate in the planning, conducting, and scheduling of timely in-service training classes that ensure a well-educated dietary service department.
• Maintain current knowledge in clinical practices of nutritional therapies and treatment protocols.
• Ensure that all dietary personnel are aware of the Care Plan and that Care Plans are used in planning daily dietary services for the residents.
• Review nurses’ notes and meets with Direct Care Staff to determine if the Care Plan is being followed.
• Discuss problem areas with the Director of Nursing.
• Review and revise Care Plans and assessments as necessary, at least quarterly and annually.

EXPERIENCE

• A degree in Dietetics or a related field is required; completion of an accredited internship is preferred.
• Previous experience working in a hospital/long term care setting is highly desirable.
• Strong knowledge of physiology as it relates to nutrition and health management.
• Familiarity with dietary systems used for patient management within healthcare facilities.
• Excellent communication skills for effective interaction with patients, families, and healthcare team members.
• Ability to work collaboratively in a fast-paced environment while maintaining attention to detail.
• Other duties may be assigned.

SCHEDULE

• 8 hour shift - 9am-5pm
• Day shift
• Monday to Friday
• Expected hours: 40 per week
• Pay Range: $50-55/hr
