POSITION SUMMARY:
The Family Partner serves as a key member of the Integrated Behavioral Health (IBH) team at GRMDC, supporting patients of all ages with a particular focus on pediatric care. This role centers on building trusting relationships with families to provide peer support, advocacy, and assistance navigating behavioral health services and community resources. Working closely with primary care and behavioral health clinicians, schools, and community agencies, the Family Partner helps reduce barriers to care, promote emotional well-being, and support child development. While not providing clinical treatment, they empower families to engage in care and connect to appropriate services, with a strong emphasis on cultural competence and collaboration across systems.

ESSENTIAL RESPONSIBILITIES / DUTIES:
- Assist patients, and/or families in understanding behavioral health systems, school supports, and community resources in addition to coordinating services through transitions in care, referrals to specialty services, and follow-up visits as needed.
- Provide education on mental health screenings, conduct preliminary screening and assessments of patient and family needs, including child development, behavioral health, and strategies for caring for children, adolescents, and adults with emotional, behavioral, or developmental challenges.
- Support patients and families with follow-up appointments, prescription requests if needed, and completing applications for community services such as food, housing, safety, and financial support.
- Connect and support patients and families to internal and external resources, helping overcome barriers to care.
- Be available for onsite consultations, with and without IBH clinicians, and actively participate in TEAM UP/Building Connections integrated care team meetings and case reviews.
- Serve as a bridge between patients, families, and clinicians to improve communication and engagement, and closely collaborate with clinicians, case managers, and community partners for coordinated care.
- Engage in outreach activities, community events, presentations, and development of patient/family support materials.
- Assist in the development and implementation of care plans for patients, record progress toward goals and document activities in the EHR.
- Empower patients and families to take an active role in decision-making about their care and hare patient and family perspectives to inform and improve program design and services.
- Advocate for culturally responsive, patient- and family-centered services within healthcare, school, and community systems.
- Provide peer support, advocacy, and navigation services to build trusting, supportive relationships with patients of all ages.

JOB REQUIREMENTS
REQUIRED EDUCATION AND EXPERIENCE:
High school diploma or GED
Or equivalent combination of education and experience.

Post- high school/GED training
1 -3 year of experience directly related to the duties and responsibilities specified. Bilingual language capacity in Albanian, Spanish or Haitian Creole.

Driver’s License, verifiable good driving record and reliable transportation.
Work towards attaining one of the CHW certification or CFP Certification within 6 months of hire.
