Summary
Responsible for obtaining referrals and facilitating the patient admission process through clinical assessment, benefit verification, patient/family/community education and interdisciplinary collaboration. Contributes to the collection and analysis of key performance indicators attributed to the admissions process.

Job Duties
  • Obtains referrals and initiates clinical review to ensure medical necessity.
  • Conducts thorough assessment of all clinical documentation and information gathered through patient/caregiver interviews to determine eligibility for admission.
  • Educates potential patients/ families and community on the benefits of LVHN post-acute rehabilitation services.
  • Establish networking connection with referral sources through ongoing unit visibility, communication, and education.
  • Verifies patient's insurance, identifies insurance plan benefits, and obtains pre-authorization if required. Completes the insurance process including all appropriate documentation within the Electronic Medical Record.
  • Ensures all regulatory requirements associated with the admission are met prior to transfer to facility.
  • Efficiently coordinates all patient transfer activities in collaboration with case management, physicians and/or nurses.
  • Responsible for bed management
  • Contributes to the collection and analysis of key performance indicators attributed to the admissions process.

Minimum Qualifications
  • Master’s Degree or Bachelor's in Physical Therapy or
  • Master’s Degree Occupational Therapy or
  • Master’s Degree Speech Therapy or
  • Associate’s Degree Registered Nurse
  • 3 years Healthcare related clinical experience.
  • Excellent problem solving skills.
  • Ability to comprehensively assess patient's appropriateness for post-acute rehabilitation.
  • Strong clinical data analytic skills measuring process and outcomes in healthcare.
  • Strong interdisciplinary communication skills.
  • American Heart Association Basic Life Support - State of Pennsylvania Upon Hire
  • RN - Licensed Registered Nurse_PA - State of Pennsylvania Upon Hire or
  • PT - Licensed Physical Therapist - State of Pennsylvania Upon Hire or
  • OT - Licensed Occupational Therapist - State of Pennsylvania Upon Hire or
  • SLP - Licensed Speech Language Pathologist - State of Pennsylvania Upon Hire

Preferred Qualifications
  • Doctorate Physical Therapy / Occupational Therapy or
  • Master’s Degree Master's of Business / Master's of Healthcare Administration or
  • Master’s Degree Master's of Nursing
  • 1 year Inpatient Rehabilitation experience and
  • 1 year Clinical Liaison experience
  • EMR Experience
  • Insurance Authorization Experience

Physical Demands
Lift and carry 40-50 lbs. Examples: Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
