Overview
Provides direct nursing care in a healthcare facility, managing a caseload of patients according to established procedures and protocols. Utilizes clinical judgment to triage and direct patients to the appropriate level of care. Supports communication with patients, families, and other healthcare providers. Contributes to maintaining standards for patient care and collaborates with other team members to ensure optimal patient outcomes.
Required Skills and Abilities
1. Licensure as a Registered Nurse and current CPR certification.
2. Excellent written, oral, and interpersonal communication skills.
3. Strong organizational and clinical skills with the ability to work under the direction of clinicians.
4. Ability to work with acutely and chronically ill patients across multiple disciplines.
5. Proficiency in using electronic medical records and basic software such as Microsoft Office.
Preferred Skills and Abilities
1. Bachelor's Degree in Nursing.
2. Experience in ambulatory care, case management, or care coordination.
3. Experience with EPIC or other electronic medical records systems.
4. Experience with endoscopy procedures or conscious sedation.
5. A minimum of 2 years of nursing experience.
Principal Responsibilities
1. Performs physical assessments of patients, including triage duties and advises on treatment or course of action by having knowledge of nursing care standards and principles. 2. Develops, implements, and manages nursing care plans in collaboration with physicians. 3. Interviews patients and records medical history to establish data for treatment plan. 4. Plans and coordinates the admission and discharge of all patients. 5. Instructs patients and families in proper treatment and advises patients on health maintenance and disease prevention. 6. Performs patient care procedures by following appropriate policy and procedure. 7. Records patients’ medical histories and symptoms, operates medical machinery, administers treatment and medications, and coordinates patient follow-up and rehabilitation; assures all procedures for maintaining patient confidentiality are maintained per HIPAA guidelines. 8. Identifies education needed to assure competent delivery of patient care for oneself and others in the department. 9. May perform other duties as assigned. Required Education and Experience Graduation from accredited school of nursing. Required License(s) or Certification(s) Licensure as Registered Nurse in the State of Connecticut or Connecticut license eligible. Current CPR certification.
