Position Summary:
The PACE Site Director is responsible for the organization, administration, development, planning, implementation, and evaluation of PACE operations for the PACE Center. The Site Director is responsible for day-to-day operations of the PACE Center and the coordination of all care delivered to participants in the PACE Program including management of the Interdisciplinary Care Team (IDT) and oversight of the facility’s operations and related systems. The Site Director also drives key outcomes for the PACE Program, such as staffing, quality, compliance, patient safety, financial performance and relationship management with contracted providers while ensuring regulatory compliance with all Centers for Medicare and Medicaid Services (CMS) and Commonwealth of Kentucky guidelines and managing the state and federal relationships surrounding the PACE Program. This position plays a key leadership role, interacting with all levels of PACE staff, participants, caregivers, providers, community agencies, health network management, executive leadership, and State and Federal representatives.

Position Duties & Responsibilities:
▪ Responsible for successful and efficient day-to-day operations and oversight of all components of the PACE Program, comprised of a primary care medical clinic, 24-hour care coordination, preventive services, a provider and healthcare network that encompasses all Medicare and Medicaid required services, therapeutic services, a PACE Center, home care, transportation, and Quality Assurance and compliance activities. Directs overarching PACE operational performance, including staffing, quality of care, safety, participant satisfaction, participant outcomes, and internal and external relationships.
▪ Delivers excellent customer experience to participants and caregivers through an efficient and effective PACE care planning and service delivery model.
▪ Ensures competent staff perform their functions and are trained in all PACE policies and procedures.
▪ Facilitates IDT meetings for developing and implementing care plans and coordinating the delivery of PACE services to meet the physical, mental, and emotional needs of PACE participants.
▪ Leads team staff meetings and other meetings as required to evaluate center operations and ensure adherence to federal and state regulatory requirements.
▪ Shares information, knowledge, concerns, and expertise through staff meetings, in-service training, and assisting in the orientation and education of new employees.
▪ Ensures accurate reporting to support quality, financial and operational initiatives, and all state and federal requirements.
▪ Facilitates the delivery of PACE required services to participants employing the highest standards for quality, meeting each participant’s physical, mental, and emotional needs through effective management of the IDT, including oversight of the care planning process.
▪ Plans and directs all marketing and business development activities for the site with the goal of growing enrollment through community outreach and an effective intake process.
▪ Responsible for the development and administration of a full and complete PACE provider network covering all PACE required services.
▪ Assists the Executive Director/AVP in short-term and long-term planning. Assists in developing strategic and business plans and routinely reviews against performance.
▪ Serves as a liaison between the PACE site’s program and other organizational programs and support services including but not limited to quality/compliance, risk, legal and facilities.
▪ Responsible for the financial performance of the site’s program. Assists the Executive Director/AVP in completing an annual operating and capital budget, reviews monthly financial statements and researches variances.
▪ Develops, manages, and monitors appropriate quality assurance and safety systems and programs collaboratively with quality, risk, and compliance departments.
▪ Ensures that the program complies with all Centers for Medicare and Medicaid Services (CMS) guidelines. Ensures compliance with federal and state regulations and licensing, accreditation, and certification requirements of all departments.
