We're seeking a part-time passionate BC/BE Internal Medicine physician to join our team of experienced practitioners and specialists delivering exceptional patient care to older adults.
Schedule: Mondays 4 to 5 hours at Applewood Village and Tuesdays, 5 to 6 hours at Pines Village

Essential Functions:
- Provide comprehensive medical care and management for sub-acute residents, with occasional support for Assisted Living (AL), Long-Term Care (LTC), and Independent Living (IL) residents as needed.
- Conduct thorough geriatric assessments to evaluate physical, cognitive, functional, and other aspects of health, tailoring care plans to meet individual needs.
- Manage complex medical conditions commonly associated with aging, ensuring patient safety, comfort, and quality of life.
- Collaborate with interdisciplinary teams, including Nurse Practitioners, nurses, and allied health professionals, to develop and implement personalized care plans.
- Communicate proactively with families, addressing concerns and providing updates on patient progress.
- Educate patients, families, and caregivers on health issues, disease management strategies, medication management, and preventive health measures.
- Comply with regulatory requirements, documentation standards, and ethical principles specific to the nursing home environment.
- Handle admissions and quarterly reviews as part of ongoing patient management.
Requirements:
- MD with a valid medical license in New Jersey (Board certification or eligibility in Internal Medicine required).
- 2+ years of relevant experience in primary care or geriatric medicine.
- Demonstrated excellence in bedside manner, fostering positive interactions with patients, families, and staff.
- Strong clinical and communication skills, with the ability to work collaboratively within an interdisciplinary team.
- Familiarity with the unique environment and regulations of nursing homes.
- Local travel may be required.
