Job Overview

The IPA Nurse RN, Inpatient Case Manager plays a key role in the care team by supporting daily care coordination for patients in the hospital or skilled nursing facility (SNF). This position involves initiating proactive case management activities for hospitalized or SNF-admitted patients in person and over the phone. They assist in transitioning patients through the care delivery system as needed, working closely with the Medical Management Team and as a member of the interdisciplinary care team to ensure effective care coordination and patient discharge planning to various settings such as home, SNF, or Long-Term Care Facility. The case manager coordinates with IPA clinics, schedules post-discharge visits with the IPA Clinical Care Team and Primary Care Provider, while also supporting HEIDS gap in care closure, Medication Reconciliation, and addresses social determinants of health as needed, enhancing the overall patient care experience.

Minimum Requirements

1. Graduation from an accredited nursing program.

2. Current valid License as a Registered Nurse through the California Board of Registered Nursing;

3. Bachelor’s degree in social work, nursing, or another health or human services field with the appropriate licensure preferred

4. Current CPR/First Aid training required.

5. Minimum of 3 years of experience in public health nursing, acute care, case management and/or home health care required.

6. Minimum of 2 years of managed care experience in case management with focus in inpatient and/or outpatient ambulatory care preferred.

7. Strong familiarity with local and regional care management resources preferred (Enhanced Care Management, Complex Case Management, Community Support Services, etc.).

8. Familiarity with case management and care delivery best practice protocols.

9. Experience and familiarity with Utilization Management (UM) standard processes and use of clinical criteria (e.g., MCG guidelines).

10. Experience with data analytics software preferred (Cozeva, Tableau).

11. Experience with physically or mentally impaired adults and/or geriatric population and working as part of a multi-disciplinary team.

Compensation

$97,410.56 - $121,763.20 annually
