Job Description: Nurse Practitioner – Healthcare Partnership Program  

Job Title: Nurse Practitioner  

FLSA Exemption Status: Non-Exempt  

Organizational Alignment: Reports to your respective HPP coordinator 

 

Position Summary: As a Nurse Practitioner in the Healthcare Partnership Program (HPP), you will be placed within a partner organization to provide direct patient care to employees and their families. This role focuses on primary, proactive and preventive care, managing chronic conditions, and addressing urgent health needs for the company’s employees, spouses, and dependents. You will work autonomously, collaborate with HealthBar’s broader healthcare team, and contribute to enhancing employee well-being through proactive health services.   

 

Essential Functions: 

  • Provide advanced clinical support to HPP and school patients, including acute and episodic care, primary preventative care, and chronic disease management. 
  • Collaborate with HPP nurses on claims analysis. 
  • Establish clinical pathways and treatment protocols for patients. 
  • Conduct telehealth visits for HPP patients. 
  • Coordinate care with the patient’s outside medical network. 
  • Act as a consultant in clinical settings, maintaining and promoting standards of care. 
  • Maintain required licensure and certifications for the role. 

 

Qualifications Required: 

  • Education: Graduation from an accredited nursing program (Master's degree in Nursing). 
  • Licensure: Active and unrestricted State of Michigan issued Registered Nurse License. 
  • Certification: Active and unrestricted FNP National Board Certification from one of the following organizations: 
  • American Academy of Nurse Practitioners 
  • American Nurses Credentialing Center 
  • Active and unrestricted State of Michigan-issued Registered Nurse and Nurse Practitioner Specialty Certification License. 
  • Experience: Minimum of 2 years providing care as a nurse in applicable areas such as med-surg, ER, ICU, acute care, public health, school nursing
