Licensed Practical Nurse

Starting Pay: $36 - $38 per hour

Hours: Part-time (30 hours per week)

This is an excellent opportunity to advance your nursing career in a dynamic and fulfilling environment. You’ll play a vital role in delivering nursing services, training, and support to meet the medical and psychiatric needs of individuals with brain injuries, intellectual and developmental disabilities, substance use disorders, and emotional or behavioral challenges. The work is both impactful and rewarding, offering variety and meaningful challenges.

Key Responsibilities:

This is a hybrid role requiring both remote and in-person work, with travel between Springfield, Hollister, Stockton, Humansville, and Nevada, Missouri.
• Conduct comprehensive medical assessments for individuals referred to the program and collaborate with intake staff for appropriate community placement.
• Develop, implement, and supervise the medical components of individualized service plans in partnership with physicians.
• Ensure nursing care plans and medical documentation remain up to date and compliant with regulatory and corporate standards, while also overseeing proper documentation, supply management, and follow-up care independently on a regular basis.
• Oversee medication administration, including documentation, compliance checks, and staff training.
• Monitor and manage high-risk medical conditions and coordinate private duty nursing services as needed.
• Maintain regular communication with primary care providers and specialists; attend medical appointments when appropriate.
• Promote health and wellness, implementing preventive strategies tailored to individual needs.
• Provide medical training and support to staff, mentors, respite providers, and, when applicable, family members.
• Present medical content during pre-service orientations and ongoing staff education sessions.
• Participate in the development and maintenance of Individual Health Plans (IHP) or Plans of Care (POC), including protocol creation and medical evaluations.
• Offer technical support regarding home care practices and the use of durable medical equipment.
• Perform additional duties as assigned.
This is a hybrid role, requiring both remote work and in-person responsibilities. As part of this position, travel between multiple sites is essential to ensure comprehensive patient care and collaboration with healthcare teams. Candidates must be willing and able to travel between Springfield, Hollister, Stockton, Humansville, and Nevada, Missouri, as needed.

QUALIFICATIONS
• Accredited RN degree, current state RN license, and current CPR/First Aid certification.
• One year of experience working with medically complex individuals.
• Current driver's license, car registration, and auto insurance.
• Ability to effectively communicate with staff, individuals served, and families.
