Senior Doc, part of Brigade Health, is seeking a Primary Care Physician (MD / DO) to deliver mobile primary care to older adults in home and senior living settings.

Primary Care Physician Responsibilities
- Provide mobile primary care services to geriatric patients in homes, assisted living, and SNF settings
- Manage and grow a patient panel
- Deliver care for chronic conditions, dementia, and complex geriatric patients
- Collaborate with interdisciplinary teams to coordinate care
- Develop relationships with facility partners and caregivers
- Maintain timely EMR documentation
- Participate in quality improvement and clinical leadership initiatives
Primary Care Physician Qualifications
Required
- MD or DO degree from accredited medical school
- Board Certified or Board Eligible in:
- Internal Medicine
- Family Medicine
- Geriatrics
- Active State Medical License or ability to obtain
- DEA License
Preferred
- Experience in:
- Home-Based Primary Care
- Skilled Nursing Facilities (SNF)
- Assisted Living
- Geriatrics
- Post-Acute Care
- Value-Based Care Models
- Experience with Medicare Advantage populations
- Leadership interest or experience
