Our team of specialists works together to guide each patient through every step of the cancer experience—from diagnosis through survivorship.</p> <p><strong>Position Overview</strong></p> <p>The Oncology Nurse Navigator serves as a dedicated care coordinator throughout the cancer care continuum. This role assesses and addresses the physical, emotional, and social needs of patients, while working closely with healthcare providers and internal/external partners to remove barriers to care. ONNs provide education, improve access, and streamline communication to ensure a supportive, seamless experience for patients and their families.&nbsp;</p> <p><strong>Key Responsibilities</strong></p> <ul> <li>Assess, plan, facilitate, and coordinate care to meet patients’ physical, emotional, and social needs</li> <li>Advocate for timely, high-quality care by guiding patients through the healthcare system</li> <li>Coordinate appointments, referrals, and services to ensure continuous care</li> <li>Provide education on treatment plans, symptom management, and health maintenance</li> <li>Maintain accurate documentation and tracking of patient progress and clinical outcomes</li> <li>Reduce barriers to care through anticipatory guidance, insurance coordination, and community resource navigation</li> <li>Collaborate with oncologists, specialists, and support teams to enhance patient experience and outcomes</li> <li>Educate patients on individualized care plans to reduce unnecessary utilization and improve quality of life</li> <li>Serve as a trusted resource for patients and families throughout the cancer journey</li> <li>Perform RN scope duties, including telephone triage and clinical assessments</li> <li>Participate in clinic activities and assist in care coordination and planning</li> <li>Perform other duties as assigned</li> </ul> <p><strong>Qualifications</strong></p> <p><strong>Education &amp; Experience</strong></p> <ul> <li>Graduate of an accredited nursing program (required)</li> <li>At least 1 year of nursing experience (required)</li> <li>Bachelor’s degree in nursing (preferred)</li> <li>Outpatient nursing and scheduling experience (preferred)</li> </ul> <p><strong>Credential
