Position Summary

Calling all superheroes in disguise! Our Social Workers are the ultimate advocates, emotional support champions, and educational wizards for our residents and their families. Join our league of extraordinary caregivers and be part of an unstoppable interdisciplinary team!

Your Exciting Mission (should you choose to accept it):

• Become the ultimate service coordinator, orchestrating a symphony of care providers (dental, optometry, referrals, etc.) like a maestro!
• Transform into a family whisperer, providing top-notch counseling and emotional support that would make even the most well known Docs jealous.
• Channel your inner Sherlock to investigate, intervene, and document resident behavior mysteries with unparalleled finesse.
• Unleash your inner psychic (or just use your awesome assessment skills) to evaluate and tend to residents' psychosocial needs.
• Don your cape as the Discharge Planning Dynamo, ensuring smooth transitions for our beloved residents.
• Become a financial Fairy Godparent, waving your wand to help residents access monetary benefits they're entitled to (no actual wand provided, but your expertise is magical enough).
• Flex your documentation muscles and become the Paperwork Ninja, mastering all resident-related Social Services records.
• Be prepared for surprise adventures - other duties may pop up faster than you can say "Social Work Superhero"!

Qualifications

Basic Qualifications & Experience

• Bachelor of Science in Social Work or equivalent work experience.
• 1 - 2 years experience in social work, preferably in a long-term care facility.
• Experience with computer word processing and database programs.
• Must be able to read, write and speak the English language.
• Must possess excellent customer service skills

Working Conditions / Physical Requirements

• Light physical effort required by handling objects up to 20 pounds occasionally and/or up to 10 pounds frequently.
• Sits, stands, bends and moves intermittently during working hours.
• Is subject to frequent interruptions.
• Interacts with residents, family members, visitors frequently.
