If you’re passionate about community work, we invite you to join our team as a Community Health Worker, where you will play a key role in the Homeless Healthcare Collaborative team by promoting greater health equity and clinical outcomes for people experiencing homelessness. Primary duties will include:

Help patients navigate and access community services, resources, and adopt healthy behaviors.

Support providers and team members with care management and community outreach.

Engage with homeless individuals in encampments, shelters, interim housing sites, health fairs, and other locations.

Promote, maintain, and improve the health of unhoused patients.

Provide social support and informal counseling to homeless individuals.

Advocate for the health needs of homeless communities.

Share the responsibility of driving the mobile clinic van. 

Note: Position is subject to the California DMV's "Pull Notice System" and continued employment is contingent upon proof of a satisfactory driving record.

Salary Range: $23.90/hour - $44.59/hour


Job Qualifications

Required:

Valid, California driver's license. Position is subject to the California DMV's "Pull Notice System" and continued employment is contingent upon proof of a satisfactory driving record.

Basic Life Support (BLS) Certification from the American Red Cross or American Heart Association.


Preferred:

Lived experience of homelessness.

Fluency in Spanish.

Experience navigating or helping people navigate medical clinic, medical insurance, and social services.
