Job Description

The Center for Health Equity & Community Wellness (CHECW) seeks to eliminate racial and other inequities resulting in premature mortality. With an unwavering grounding in history and structural analysis, CHECW works to increase visibility of the harm perpetuated by centuries of racist, socially unjust policy while pushing towards redress for the most impacted NYC communities. CHECW addresses inequity across community and healthcare systems in partnership with community, faith-based, and environmental and commercial determinants, and addresses both upstream and downstream factors to improve health and well-being of New Yorkers.

CHECW is comprised of the Bureau of Bronx Neighborhood Health, the Bureau of Brooklyn Neighborhood Health, the Bureau of Harlem Neighborhood Health the Bureau of Chronic Disease Prevention, the Bureau of Health Equity Capacity Building, the Bureau of Equitable Health Systems and the Bureau of Finance, Administration and Services. The division's Deputy Commissioner also serves as the Agency's Chief Equity Officer and oversees Race to Justice, the Agency's internal reform effort to help our staff learn what they can do to better address racial health gaps and improve health outcomes for all New Yorkers. The division's Deputy Commissioner also serves as the Agency's Chief Medical Officer. The Office of the Deputy Commissioner and Chief Medical Officer seeks to hire a Director of Provider & Community Engagement .

Reporting into the Office of the Deputy Chief Medical Officer, the Director of Provider & Community Engagement will lead development and implementation of strategies for leveraging a comprehensive Provider Data Warehouse to contain master records of NYC healthcare providers based on merging data from multiple agency sources/databases in order to better engage and communicate with providers.

DUTIES WILL INCLUDE BUT NOT BE LIMITED TO:
Reporting into the Office of the Deputy Chief Medical Officer, the Director of Provider & Community Engagement will: Lead development and implementation of strategies for leveraging a comprehensive Provider Data Warehouse to contain master record of NYC healthcare providers based on merging data from multiple agency sources/databases in order to better engage and communicate with providers. Health care providers are critical to supporting COVID-19 vaccinations, myth busting, prevention, and treatment. In partnership with the Provider Informatics Manager, the Director of Provider & Community Engagement will support creation and use of centralized provider data warehouse with provider information across provider type, location, demographics, specialty, etc. The Director will work collaboratively with the Director of Strategy and Partnerships to improve provider communication and outreach. Research, develop, implement, and strategized for enhanced engagement with diverse health care providers across New York City, including primary care clinicians, specialty care clinicians, and healers (acupuncture, naturopathy, etc.). Research and define success criteria and key performance indicators (KPIs; e.g., open rates, click-through rates) for ad-hoc communications and planned campaigns. Recommend and test communication strategies to increase provider engagement. Supervise staff members (Provider Engagement and ICS Coordinator, Product Manager for Caremesh). Research and develop a thorough understanding of the provider population, across various strata including provider specialties and populations served.
Research and develop a thorough understanding of the community health worker workforce and develop strategies for training and engagement across the agency. Evaluate various data sources, including community and partner surveys, landscape assessments, and local and national data to assess emerging needs and priorities. Use evaluation findings to inform program development and recommendations, collaborating with cross-agency leadership. Review literature to assess for innovative and effective provider and community engagement strategies. Work with Bureau in the research and writing to publish papers on lessons on community engagement approaches. Collaborate with agency leadership to develop, implement, and evaluate strategic alignment of Public Health Corps (PHC) programming across the agency. Oversee PHC reporting internal and external to DOHMH. Serve as program lead to integrate PHC and Community Health Worker programming with Agency and Divisional priorities such as Healthy NYC and Chronic Disease Prevention Plan. Oversee and continue management of Health Advocacy Partners Program, directly managing the coordinator of HAPP and also overseeing the development and management of CHW ambassador program.

PREFERRED SKILLS:
Master's degree in a related area (MPH, MSW, MSc, etc.)
