About the role

  • Evaluate patients within 24 hours of physician referral and document any delays; develop and implement treatment plans with physician approval.

  • Provide direct patient care in accordance with treatment plans; supervise and guide Physical Therapy Assistants and Restorative Aides.

  • Participate in care conferences, utilization reviews, and in-service training for staff; assist in discharge planning and caregiver training.

  • Maintain detailed and timely documentation, including evaluations, treatment notes, progress notes, and recertifications per state and payer requirements.

  • Communicate effectively with supervisors, healthcare team members, patients, and families regarding patient progress and care plans.

  • Recommend, order, and facilitate necessary durable medical equipment (e.g., walkers, braces) to support patient mobility and independence.

  • Ensure all therapy equipment is properly maintained; report and address any equipment issues promptly.

  • Maintain current Physical Therapist licensure and meet ongoing education requirements per state regulations; proficiency with Casamba and Point Click Care is preferred.

  • Perform physical tasks regularly, including lifting over 50 lbs, standing, walking, pushing/pulling, and occasional stooping or kneeling.

  • Foster positive, effective relationships with patients, families, staff, and colleagues; uphold all policies regarding safety, confidentiality, and professional standards.
