JOB SUMMARYThe VHAN Clinical Outcomes Specialist utilizes existing reports and collaborates on the development new reports that identify population health aggregate trends and ensures their incorporation into workflows as appropriate. This role also collaborates on the development of strategies to mitigate negative trends in financial or clinical performance to improve quality metrics and promote optimal clinical outcomes.

KEY RESPONSIBILITIES
- Collaborate with enterprise analytics to apply clinical context to data trends and analysis.
- Ensure payer and other reports are reviewed and prioritized for clinical impact, assisting with process design to incorporate into care management workflow.
- Based on data trends and in-depth clinical reviews, advise care and utilization management leadership team of recommendations for changes in practice to effectively utilize care management outreach and other programs as available most effectively.
- Assist in the development strategies to impact utilization KPIs based on clinical knowledge and data trends.
- Support program development by applying knowledge of clinical standards and best practices to determine appropriate utilization of human resources and other tools for patient engagement (home-based services, remote patient monitoring, digital outreach, etc.) in partnership with case and utilization management leadership.
- Provide clinical support to various utilization workgroups, including but not limited to ED utilization, Readmissions, and Total Cost of Care.
- Serve as a clinical liaison for various VHAN partnerships as needed.

Demonstrates the ability to sift through data to determine its most significant elements. Able to turn discrete data into meaningful and valuable information which contributes to the successful completion of job assignments. Clearly recognizes cause and effect relationships.
