Job Description for Community Director

Position supervises: Clinical Manager, Community Assistant

Position Summary: The Community Director is responsible for programs offered in assigned Community, which may include hospice care for adults and pediatrics, palliative care, and possible future lines of business.
The Community Director ensures the delivery of premier care to patients and families through an interdisciplinary team of clinicians. The Community Director manages all aspects of operations, including quality and clinical excellence, financial outcomes, business growth and community partnerships, regulatory compliance, and mentoring managers.
Position Duties & Responsibilities:
• Achieves the strategic outcomes of the community, including quality outcomes, financial results, growth and employee engagement.
• Achieves territory and organization goals through collaborative relationships with community leaders, clinical managers, and Hosparus Health support departments.
• Assures cost effective and clinically effective care and assists with developing, implementing and evaluating current and new programs and services.
• Manages community relationships and collaborates with outreach team in performing marketing and outreach initiatives.
• Maintains knowledge and compliance with organizational policies, state and federal regulations, and accreditation standards pertaining to hospice care and other lines of business as applicable per community.
• Assures utilization of performance improvement principles and practices.
• Partners with the quality department to ensure continuous survey readiness and compliance with all applicable requirements.
• Provides guidance, coaching, and mentoring to managers/supervisors in the day-to-day clinical operations related to care, compliance, staff education, safety, etc.
• Assesses staff adherence to expectations and holds staff accountable for following Hosparus Health Visit Design model, delivering quality care and services, and achieving Individual Measures of Success.
• Responsible for implementing, evaluating and maintaining community accountability for organizational programs related to improving quality; monitors quality and data outcomes to improve quality metric scores and overall quality of care; reports outcomes to the VP of Operations.
• Schedules and performs QPRs/audits to monitor and evaluate quality of care.
• Works closely with the compliance department to support regulatory and compliance activities; monitors plans of correction (PPOCs) for resolution of deficiencies identified in survey preparation or the survey process.
• Develops and maintains a quality monitoring system to continuously monitor and measure quality, safety and clinical outcomes for the community.
• Assists in the preparation of annual budget including census projections.
• Reviews monthly financial performance by reviewing financial statements and outcomes, with particular attention to payroll efficiencies and cost-effective care delivery. Adjusts staffing and processes accordingly.
• Develops, implements and monitors staffing plans based on organizational guidelines and in a fiscally responsible manner to meet patient needs and plan for census growth.
• Ensures overall community productivity including staff productivity and, in addition, efficiency related to processes and procedures.
• Develops relationships with community partners and referral sources; explores opportunities to initiate, grow or maintain relationships and ensure a high quality of service to those partners. Initiates service recovery as needed.
• Participates in admissions or clinical manager on call rotation.
• Other duties, as assigned.
