💼 What You’ll Do

As an RN Care Coordinator, you’ll deliver skilled nursing care to patients in the comfort of their homes—managing the full clinical cycle from admission through discharge.

Key responsibilities:

• Conduct initial and ongoing nursing assessments

• Develop, implement, and revise individualized plans of care

• Deliver direct nursing care and specialized interventions

• Educate and counsel patients and their families

• Document visits using OASIS and ICD-10 with timeliness and accuracy

• Coordinate multidisciplinary care across physical therapy, speech, social work, and more

• Supervise LPNs and HHAs, and participate in QA and peer review initiatives

• Participate in on-call rotation for nights, weekends, or holidays

✅ You’re a Great Fit If You:

• Hold an active RN license in [State]

• Have 2+ years of nursing experience (home health preferred)

• Possess strong clinical judgment and independent decision-making skills

• Are highly organized, communicative, and calm under pressure

• Are passionate about delivering patient-centered care at home

• Embrace technology for documentation and care coordination
