In this role, you’ll provide thoughtful, one-on-one care with the time and focus patients deserve; no rushing from case to case. We’re seeking a self-directed nurse who takes initiative and wants to be part of a team that’s rethinking home care. If you’re looking for flexible, purposeful work we’d love to hear from you.

Requirements: 

• Unencumbered New York State RN License
• CPR, BLS Certification
• 2 or more years of hospital experience (Acute Care) and/or Home Care
• Ability to commute to Brooklyn, Manhattan, Queens, and the Bronx
• Willingness to be part of a growing Team

Duties:

• Perform comprehensive in-home assessments 
• Develop or update Care Plans 
• Perform Supervisory Visits 
• Communicate with the office, caregivers, and clients/families
• Provide ongoing education and in-field support to HHAs and PCAs
