Your Specific Responsibilities:

This is a professional OPS registered nurse position in the field of public health responsible for providing family centered care coordination to a population of HIV positive pregnant women and HIV exposed/positive babies in Area 15 (St Lucie, Martin, Okeechobee, Indian River).  This position will be the lead Hug Me Nurse for Area 15. This includes assessing, planning, implementing, coordinating, monitoring and evaluating services to meet the needs of the identified population.  Performs work in an area geographically removed from direct supervision, i.e. Home, hospital, doctor’s office, clinic settings, labs, etc.  The incumbent will comply with all DOH policies and procedures regarding the security of confidential information.  The incumbent works directly under the supervision of the Community Health Nursing Consultant.  Use of personal vehicle may be required. Duties include, but are not limited to:

Accepts primary responsibility for coordinating the management of care and ensuring follow-up by:

a. Working collaboratively with other disciplines, e.g. Healthy Start, TOPWA, Ryan White case management, HIV Surveillance, DIS, OB’s, pediatricians, ID specialists and Pediatric ID specialists in assessing and meeting the physical, psychological, social, emotional, and rehabilitation needs of HIV exposed infants.

b. Providing nursing consultation to children and their families, other team members and the community, regarding management of the health needs of the patients, assuring continuity of care.

c. Reviewing and assessing laboratory and other diagnostic tests and reports.

d. Planning and implementing actions, identified along with the family, to meet the needs of the patient.  Monitoring and evaluating outcomes and goal achievement.

e. Completing established assessments and care coordination plans

f. Teaching parents/guardian’s skills and information needed to care for the child in the home as outlined in CDC guidelines

h. Identifying and utilizing appropriate DOH and other community resources to meet the needs of the children and their families.

 i. Providing education and support to families with children with special health care needs to increase the family’s capacity to become the child’s primary care coordinator.

i. Making home, hospital visits for coordination and continuity of patient care.  Attending client staffing’s as needed.

j. Assisting families and individuals to cope with crisis situations such as chronic illness, surgeries, and terminal illness.

k. Coordinating the provision of care utilizing the available ID specialty pediatric care

l. Providing health education to women of childbearing age, those who know their HIV status and those whose status is unknown, regarding the importance of being tested prior to delivery and prevention of perinatal transmission

m. Educate HIV infected mothers about the importance of ongoing follow up care and adherence to medication therapy for their exposed infants and themselves

n.  Meet with HIV infected mothers before they are discharged from the hospital

o. Ensure that sufficient medication is available for the infants upon discharge

p. Link both the HIV infected mother and the exposed infant to appropriate follow up care, to include medical transportation

q. Provide home visits
